An affidavit is a sworn written statement of facts filed in Ontario family court. Judges rely on them before hearings, motions, and conferences. Use Form 14A (Affidavit, General) from ontariocourts.ca.

Step-by-Step Guide

1. Identify Yourself

State your full legal name, city, and role: "I am the Applicant in this proceeding."

2. Write in First Person

Use "I attended", "I received", "I observed." Be factual. No emotional language or opinions.

3. Number Every Paragraph

Ontario Rules require numbered paragraphs from 1. One topic per paragraph.

4. State Facts Only

No legal arguments. State what you personally saw, heard, or did.

5. Attach and Label Exhibits

Attach referenced documents as Exhibit A, B, etc. Reference each in the body.

6. Swear Before a Commissioner of Oaths

Sign before a lawyer, notary, or court staff. Bring valid photo ID.

7. Make Copies Before Filing

Three copies minimum: court, other party, your records.
